LUMP SUM DEATH BENEFIT

If your death occurs after you begin receiving your pension, a “Lump Sum Death Benefit” is payable to anyone you name.

The amount of the Lump Sum Death Benefit is $1,500. If your eligibility for a pension was based on reciprocal service with another UFCW pension plan, the Lump Sum Death Benefit amount is determined by multiplying $150 times your years of Eligibility Service under this Plan (up to a maximum of $1,500).

You may name anyone you wish as your beneficiary for the Lump Sum Death Benefit. If you name more than one person, the benefit is divided equally among those you name. At the time you retire, you will be required to complete and return a “Beneficiary Designation Card” naming the person (or persons) you wish to receive the benefit. Please keep this card up to date so that, upon your death, the benefit can be paid promptly and exactly as you wish it.
