Request Pre-Certification of Expenses

Submit a Request for Pre-Certification of Expenses for any non-emergency hospital admission (for emergency hospitalization, within 48 hours of the admission), surgery, advanced diagnostic testing, skilled nursing facility care, rehabilitation therapy, home health care, hospice care, and purchase or rental of durable medical equipment.
